Who qualifies for home health services?
Homebound individuals with chronic, severe or permanent conditions. These individuals must be under the current care of a physician, dentist or podiatrist.
Skilled Nursing Services
Qualified registered nurses and licensed vocational nurses. Individualized care plans to promote, maintain and assist in restoring health and minimizing the effects of illness and disability. Personalized education programs designed to meet the individual needs of each patient. Wound care management (including surgical wounds, pressure ulcers, static ulcers, etc.), IV Antibiotic Therapy, IV hydrations, electrolyte therapy and parental nutrition.
Administration of injections, Foley catheter care, Ostomy care, Blood glucose monitoring and insulin management. G-tube care and enteral nutrition, pre and post-surgical care, central line management. Certified home health aides provide personal care and aid in the overall improvement of the patient’s quality of life.
Physical Therapy Services
Physical therapist develop care plans that restore muscle and joint function,
assist in improving and facilitating medium self-care at optimum function.
Occupational Therapy Services
Help the patient learn to perfom physical tasks & regain skills required for independent living
Speech Pathology Services
Assist with impaired communication skills using alternative methods to regain optimum speech
Medical Social Services
Provide education techniques and models to assist with emotional impacts of the illness or disability. Providers asses and evaluate factors affecting the patients recovery, such as: financial assistance, referral to community resources and Support groups Physical therapy, occupation therapy, speech therapy, and medical social services provided by arrangement.
Referral Sources
Patients physicians-Hospital - Board and Care-Skilled Nursing Facility Patients Family - Any Interested person or agency Service will begin within 24-48 hours upon receiving a referral approved by a physician.
Method of Payment
Medicare, Medical, Private insurance, Private pay and LA Care. Other payment options may be arranged. We serve the Los Angeles area within a 50 mile radius from our office.
